Create a New Client for Supported Employment

Select Clients from the Menu bar, then select Create New Client from the drop-down menu.
Client Details
This will bring up a New Client Form.
Enter the Client's Details:
Enter the Client's Contact Information:
Enter the Client's Address:
Next, you need to complete the Client Registration:
- Alpha ID- The Alpha ID is the ID number you use to identify your client.
- Type- This indicates what Type of client you are entering, make a selection from the drop-down menu. e.g. Job Seeker.
- WINZ Number- Enter your client's WINZ Number.
- SLI Rating- An SLI Rating (Support Level Indicator) provides an indication of the type and amount of support people need prior to gaining employment and while in employment.
- PE Hours- Enter your clients Preferred Employment Hours.
- Case Worker- Select the client's case worker from the drop menu.
- Follow-up Date- Enter the date your client will require a follow-up.
- Application Status- Enter the status of their application or leave blank if no applications have been submitted.
- Phase- Select the Phase your client is in from the drop-down menu. eg. Pre-employment Activity
- Further Job Search- Select an option from the drop down menu.
- Registration Date- Enter the Registration Date.
- Service End Date- Enter the Service End date.
- De-Registration Date- Enter the date for De-Registration.
- De-Registration Reason- Select a reason for De-Registration from the drop-down menu.
- Important Information- Enter any information that you deem important to your client.
Once you have filled out the Registration Details, click on the Next button:

To add another Emergency Contact, select:
When you have completed filling out the emergency contact page, click Next:
Demographics

- Select the Birth Country from the drop-down menu.
- Select your clients' Ethnic Origin.
- Select their preferred Language.
- Select their Living Arrangement from the drop-down menu.
- Enter the Income Source for your client.
- Select the Support Office Location.
- Select the Referral Source.
- Does your client have a Disability Pension?
- Do they have a second Supplement Benefit?
- Do they receive a Youth Allowance?
- Do they receive a Mobility Allowance?

Employment
- Enter any Barriers to Employment and Challenges your client may have.
- Record your clients' Education history.
- List any relevant skills that will be of benefit to your client's employment.
- Enter your clients' Qualifications.
- List your clients' Work Experience.
- Record any interests your client has.
- Enter your client's IRD Number.
- Select from the drop-down menu how your client will get to and from their employment.
- Select which Office they are registered in.
- Select what type of driver's license they hold.
Comments
Enter any comments that are relevant to your client. Once you have completed all of the relevant fields in the New Client Form, click Submit and your new client will be saved and added to your client list.
